标题[请益] PHP新手问题(关於多个下拉式选单)

请问各位版上的高手执行时显示这个问题Notice: Undefined index
我那个位置是放一个下拉式选单我是用(GET的参数如果是用POST就没这个问题)
请问要如何解决
